COVID-19 Vaccinations at the Government Center

The City of Sunny Isles Beach in partnership with the Florida Department of Health will offer COVID-19 vaccinations for individuals ages 12+ at the Sunny Isles Beach Government Center, 18070 Collins Avenue on Wednesday, June 16 from 8 am – noon.
The vaccine offered is Pfizer, which requires two doses. Those who receive their first dose on June 16 will then receive an appointment time to receive their second dose on Wednesday, July 7. You must be available on both dates to receive the vaccine.

You must present a valid photo ID at your appointment. Minors must be accompanied by a parent or guardian. All patrons will be required to fill out and submit a Vaccine Screening and Consent Form. Print and fill out the form below to expedite the check-in process.
